Q. I am an Indian, aged 42, working in Kuwait as Safety Officer. I am married. I have a back problem to my vertebra. This is at the lower portion. I went to a Doctor (Ortho) at hospital. He took X RAY LBS portion and said there is no problem with the bones . He gave some pain killers which has not affected much .The pain still persists when I stand and sit continuously for more than half hour . Is this related to any veins or muscles. Can you advice to any Doctor for a physical check up on any Friday ?

A. Back ache is a very common problem and can be caused by a number of reasons. Most people with back ache have a normal x-ray and that does not rule out problems related to the spine. Other than spine problems , problems in the pelvis , genito-urinary system or abdomen can also produce backache. A good proportion of chronic back ache is related to faulty posture, overweight and psychological factors. Please see a specialist in orthopedics or Internal Medicine and he will be able to help you in the management. Unfortunately these specialists are seldom available on Friday and you might be able to fix up an appointment on another day.
Q. I am a 30 year old accountant. My father is a diabetic. My fasting blood sugar is 105mgs and 2 hrs after food is 187mgs. My doctor says the value after food is above normal. Do I have diabetes? Should I do anything?

A.You have a condition called Impaired Glucose Tolerance (IGT). Being the son of a diabetic, you have a genetic predisposition for the same. IGT is the stage before diabetes and is associated with complications like increased risk of heart disease etc. Studies have shown that diabetes can be prevented even if a person has IGT to an extend of 58%, by life style changes. The life style changes consist of maintaining ideal body weight, regular exercise, reducing stress, not smoking and avoiding medicines that can elevate blood sugar. If these do not control the elevated blood sugar, medicines may have to be used which are shown to reduce the progress to diabetes by 30%.
Q. I am 42 year old house wife. I was detected to have diabetes 3 months ago and was started on Glynase 5mg in the morning before breakfast. I feel very hungry around 11.30am and if I don't eat immediately I feel very weak , tremble and sweat. Why is this? What can I do?

A. Your symptoms are due hypoglycemia i.e. the blood sugar dropping below normal. Even though eating at that time gives relief, in the long run this is likely to lead to weight gain and poor control of diabetes. You should discuss the symptom with your doctor and he would probably change your medication into one that does not lower blood sugar below normal.
Q. I have been having a throbbing headache for a long time. I am 25 years old and came to Kuwait 2 years ago and work as a receptionist. The headaches have increased and sometimes I vomit also. I have seen many doctors and even had a brain scan. I have to take upto 6 Panadols per day for pain. My blood pressure is normal. Please advise as to what can be done ?

A. Your history is suggestive of a vascular headache. There may be other members in your family with the same problem. Exposure to sun, hunger, certain types food like chocolate and stress can worsen this type of headache. Try to avoid precipitating factors. Certain medicines can also reduce the frequency of this type of headache.
